Fifth Avenue Committee (“Fifth Avenue Committee”, “we”, “us”, or “our”) values your privacy. Identifiers and contact information: When you apply for or participate in our programs or otherwise engage with our Services, we may collect identifiers and contact information such as full legal name, nickname, Social Security number, tax ID numbers, email addresses, phone numbers, postal addresses, websites and social media accounts.

Financial information: When you apply for or participate in our programs, we may collect financial information such as income amount, income and financial history, credit score and credit score history, credit check history, sources of income, assets, rent and mortgage payment information, and housing financing details.

Demographic information: When you apply for or participate in our programs, we may collect demographic information such as ethnicity, race, immigration status, primary language spoken, English and other language fluency, gender, gender pronouns, sexual orientation, transgender status, marital status, housing status, housing type, number of household members (including number of dependent children), information about dependent children (e.g., education status; whether they were in foster care), employment information, employer name, education status (e.g., highest credential obtained), educational needs (e.g., learning disabilities), education and vocational training history, health insurance information (including ID number), mental and physical healthcare history (including prescriptions), veteran status, disability status, voter registration status, date of birth and age.

Program enrollment information: When you apply for or participate in our programs, we may collect program enrollment information such as registration date, service dates, programs and services you are interested in, counseling and education needs, resource requests, referral sources and how you heard about the program. Depending on the program you enroll in, we may collect additional information about your health screenings, housing needs and complaints, food security, car ownership, transportation needs and government benefits. We may additionally collect information in response to questions we ask you about domestic violence experiences, financial and employment security, childcare and day-to-day needs.

Photographs and Videos: We may take photographs and videos of you at our public events, and will post notices of the same at such events. When you participate in our programs, we may also take photographs and videos of you, with your consent.

Donation information: When you donate to Fifth Avenue Committee, we may collect donation information, such as payment card information (e.g., cardholder’s name, credit or debit card number, expiration date, security code, and billing address), bank account number and routing number, donation amount, donation frequency (i.e., one time or monthly) and information from accompanying messages. We may also collect your employer’s name.

Communications with us: When you contact us via email or otherwise, we collect the contents of your communications.

Device and usage information: When you visit our Site, we automatically collect device and usage information, such as your IP address, the browser you used, the device you used, the date and time of your visit, and what pages you visited, through cookies and other tracking technologies, including third-party cookies and tracking technologies, when you use the Site. Legal Reasons: To: (1) comply with applicable law or a valid court order, subpoena, or government mandated request, (2) participate in litigation, civil discovery or other adversarial legal proceedings, (3) enforce our policies, including this Privacy Policy and our Terms of Use, and investigate potential violations thereof or (4) protect you, us, and others from fraudulent, abusive, malicious, deceptive or unlawful activities.

Business Partners: We may disclose your Personal Information to our business partners, including NeighborWorks America, for the purposes for which you provided such Personal Information or as reasonably necessary for our joint initiatives and collaborations with such business partners.

Our Site is not intended for minors under the age of 18 and we do not knowingly collect Personal Information from minors under the age of 18. We reserve the right, in our sole discretion, to change this Privacy Policy and will update the “Last updated” date at the top of this Privacy Policy upon doing so.
