- Identifiers and contact information: When you apply for or participate in our programs or otherwise engage with our Services, we may collect identifiers and contact information such as full legal name, nickname, Social Security number, tax ID numbers, email addresses, phone numbers, postal addresses, websites and social media accounts.
- Financial information: When you apply for or participate in our programs, we may collect financial information such as income amount, income and financial history, credit score and credit score history, credit check history, sources of income, assets, rent and mortgage payment information, and housing financing details.

- Program enrollment information: When you apply for or participate in our programs, we may collect program enrollment information such as registration date, service dates, programs and services you are interested in, counseling and education needs, resource requests, referral sources and how you heard about the program. Depending on the program you enroll in, we may collect additional information about your health screenings, housing needs and complaints, food security, car ownership, transportation needs and government benefits. We may additionally collect information in response to questions we ask you about domestic violence experiences, financial and employment security, childcare and day-to-day needs.
- Photographs and Videos: We may take photographs and videos of you at our public events, and will post notices of the same at such events. When you participate in our programs, we may also take photographs and videos of you, with your consent.
- Donation information: When you donate to Fifth Avenue Committee, we may collect donation information, such as payment card information (e.g., cardholder’s name, credit or debit card number, expiration date, security code, and billing address), bank account number and routing number, donation amount, donation frequency (i.e., one time or monthly) and information from accompanying messages. We may also collect your employer’s name.
- Communications with us: When you contact us via email or otherwise, we collect the contents of your communications.
